- Criminal Background Check w/ $25 fee
- I-9 form with 2 forms of Identification
- W-4 (for current year)
- Direct Deposit Form
- Policy Agreement Form - signed
- Physical Assessment (St. Gabriel's hospital for PCA/EP, Food Service, Custodian, Cleaners, etc), if applicable.
- Contract or Agreement
- Copy of License (if applicable)
- Original Transcripts (if applicable)
- Benefit paperwork must be turned in within 30 days of hire
- Picture taken for Photo ID (Human Resources Office)
- FOB, Photo ID badge, laptop, Ipad (http://www.lfalls.k12.mn.us/departments/technology), if applicable.
- Skyward login, Email setup and login (Mary Beth Grams, http://www.lfalls.k12.mn.us/departments/technology)
- Keys for classroom (Principal), if applicable.
- Time sheets are obtained from the Building Secretary, if applicable.
